Bluebonnet Trails Community Services today announced changes to its primary care services in Fayette and Lee Counties. After careful consideration, the organization will discontinue primary care services at its Giddings location on Dec. 23, and transition ownership of its La Grange Primary Care services effective Jan. 1, 2026.
All other services at Bluebonnet Trails’ La Grange and Giddings Clinics will remain available without interruption.
To ensure continuity of care, Bluebonnet Trails is excited to announce a partnership with Progressive Health of Fayette, LLC, which plans to reopen St. Mark’s Hospital in La Grange with space for a primary care clinic, and is slated to reopen in the first quarter of 2026. Beginning Jan. 1, 2026, Progressive Health will provide primary care services at Bluebonnet Trails’ La Grange Clinic during a transition period until their new location at St. Mark’s is ready.

What Patients Need to Know:
• Letters have been mailed to all impacted clients with details and options.
• Individuals and families receiving services in Giddings are invited to schedule appointments in La Grange after Dec 23rd.
• Primary Care appointments scheduled at the La Grange location after January 1st will not be impacted.
