Columbus Oaks Healthcare Center is proud to serve the Columbus community and surrounding areas with compassionate, high-qual ity care designed to meet a wide range of medical and rehabilitative needs. Under new management as of Nov. 1, 2025, the center continues to strengthen its commitment to excellence, personalized care, and community partnership.
Columbus Oaks offers skilled nursing services, short-term rehabilitation, long-term care, and an on-site Assisted Living community, allowing residents to receive the appropriate level of care while remaining on one trusted campus. The center provides physical, occupational, and speech therapy for both inpatient and outpatient clients, giving community members access to professional therapy services without requiring a residential stay.
Specialty services include wound care, IV antibiotics administration, and comprehensive support for individuals living with dementia and Alzheimer’s disease, delivered by trained staff who prioritize dignity, safety, and individualized attention.
Columbus Oaks also accepts TPN patients (with a defined stop date) and PEG tube patients. In addition, the facility will be accepting tracheostomy patients in the near future, with admissions reviewed on a case-bycase basis to ensure the highest level of care and clinical readiness.
